How to Register
You can call into either surgery and complete the New Patient Registration Forms, or if you prefer you can down load them from the links below. Please note that registrations cannot be taken over the telephone. You will also need to provide two forms of identification when you register such as a birth certificate, passport or driving license.

Detailed coded Records
Access to your detailed coded records is now avaialble – this is not an automatic facility.
Should you require this access you can request it when you log in to your system online – your request will go to the Doctors meeting and they will endeavour to activate or get back to you within 21 days
