If you need to request a repeat prescription you should use the right hand side of the last prescription which was issued to you. You will see that there are options for where you wish to collect the prescription from and boxes alongside the drugs to be ticked to show which you require. Completed forms can then be left in either surgery or at one of the pharmacies in Wilsden, Cullingworth or Denholme.

Please allow two working days for your prescription to be prepared.
