How to Register
You can call into either surgery and complete the New Patient Registration Forms, or if you prefer you can donwload them from the link below. Please note that registrations cannot be taken over the telephone. You will also need to provide two forms of identification when you register such as a birth certificate, passport or driving license.
All newly registered patients need to book a New Patient Health Check.
